We do not ask for height and weight alone. Those two numbers cannot tell us whether someone has a longer torso, broader shoulders or prefers a looser waist.
Our sheet asks for chest, waist, hip, shoulder width and preferred top length. For trousers we ask for waist, hip, inseam and the length the staff member already wears comfortably.

We also ask two questions that are not measurements. Do you prefer a fitted or relaxed top? Do you tuck your scrub top in? Both answers change the size we recommend more than a centimetre here or there.
For teams above 25 people, we send a sample set in two sizes and let staff try them for a day. The feedback we get from that trial is usually more accurate than any form.
If you are ordering for the first time, do not aim for a perfect fit on every single person. Aim for a size set that covers 90 percent well and handle the rest with alterations or a second run. That is how real uniform rollouts work.
